There is a watch on your wrist and it matters more than it sounds. Check it every 30 seconds or so to remind yourself who you are and where you come from, because the longer you ignore it, the faster your sanity slides. When the madness meter fills, the screen bends, the voices creep in, and the run is usually over. I liked this mechanic more than I expected, since it turns a quiet walk into a countdown.
Somewhere in those rooms a Being wanders, and it does not always appear in a run, which somehow makes the threat worse. There are rules about it. Do not look at it. Do not get close. Headphones help a lot, because footsteps that are not yours are the best warning you get.
Getting lost constantly taught me to keep moving, check the watch often, and treat every new room as a place to leave quickly.
